Compounded Semaglutide Oral Suspension for Sublingual Administration
GLP-1 Receptor Agonists
The notably minimal absorption rate of oral tablets (less than 1%, according to the labeling for RYBELSUS®) has led many patients to prefer avoiding injections if possible. The worldwide utilization of GLP-1 receptor agonists has expanded significantly, encompassing several different injectable medications and one orally administered drug.
Considering these and other potentially valid reasons, prescribers and patients may opt for a patent-pending compounded formulation of semaglutide.
What is Sublingual Semaglutide and How is it Different?
Our patent-pending sublingual (SL) Semaglutide formulation is a compounded suspension of semaglutide intended for administration under the tongue. It features a specially formulated base designed to facilitate the transport of large molecules, like semaglutide, across the oral mucosal membrane. This self-emulsifying base is equipped with permeation-enhancing and distinctive mucoadhesive properties, which aid in the absorption of molecules into the mucosal tissue.
A recent laboratory study showed that our compounding base effectively delivers semaglutide into human gingival and oral tissues.
What is the Dosage and How is it Supplied?
Providers:
When titrating the dose, it’s crucial to consider that semaglutide has a half-life of about 7 days and typically takes 4-5 weeks to achieve steady-state drug concentrations. Additionally, due to daily dosing rather than weekly, patients may experience a more gradual titration and more consistent steady-state concentrations.
Patients:
If you are initiating semaglutide for the first time, your healthcare provider may advise starting with a gradual titration. This involves beginning with a lower dose and gradually increasing it over time. This approach to dosing can help minimize gastrointestinal side effects and nausea. It typically takes 4-5 weeks after a dose increase to achieve maximum medication levels.
The medication is packaged in an amber bottle with a 1ml dropper for convenient sublingual administration. Store it at room temperature and shake well before use. Administer the dose prescribed by your doctor under your tongue for at least 5 minutes, then swallow. Refrain from eating or drinking anything for at least 30 minutes after taking it./p>

What is Semaglutide?
Semaglutide has been utilized for years to assist individuals with type 2 diabetes in managing their blood sugar levels.
It is marketed by Novo Nordisk under the brand names Ozempic® as an injectable and Rybelsus® as an oral tablet, with several indications for diabetes patients. In 2021, the FDA also approved Wegovy®, another injectable drug containing semaglutide, for aiding in weight management.
Although there are specific FDA-approved indications that are available from reviewing the Prescriber Information (“PI”) included in the labeling of the respective medications, you can often look at past or ongoing clinical trials for information about the medication. Please see the Clinical Trials white paper above for more information.
Compounded Sublingual Semaglutide Suspension is a compounded medication, and the FDA does not review compounded medication for safety or efficacy.
What are the Side Effects?
The predominant side effects often involve gastrointestinal symptoms like abdominal pain, constipation, diarrhea, nausea, and vomiting. These effects may be more prevalent when starting treatment or during dosage adjustments. Immediate medical attention is necessary for severe allergic reactions such as rash, difficulty breathing, or swelling of the mouth, face, or throat. There is also a risk of low blood sugar, particularly when combined with other diabetes medications. Suspected episodes of low blood sugar may require discontinuation of the medication.
CMPD refers to compounded medication, FDA does not review compounded medication for safety or efficacy, Semaglutide is being compounded using crushed RYBELSUS tablets to obtain the Semaglutide.
